On Ozempic or a GLP-1? How to Lose Fat, Not Muscle

More women than ever are using GLP-1 medications like Ozempic, Wegovy, Mounjaro and Zepbound. I'm not here to judge anyone's choice. That's between you and your doctor. But as a coach, there's one thing I want every woman on these medications to know:

The number on the scale isn't the whole story. What you lose matters.

Why muscle loss is a real concern on GLP-1s

These medications turn down your appetite, which is a big part of why they work. But when you're eating much less, especially less protein, and not strength training, your body can lose muscle along with fat. Studies have found that a meaningful share of the weight lost on these medications can come from lean mass.

Why does that matter? Muscle keeps your metabolism up, protects your joints and bones, and keeps you strong as you age. Lose too much of it and you may end up smaller but weaker, more tired, and more likely to regain fat if you ever come off the medication.

The good news: you have a lot of control here.

5 ways to protect your muscle on a GLP-1

1. Make protein the first thing on your plate

When your appetite is low, eat your protein first. Most women do well aiming for roughly 0.7 to 1 gram of protein per pound of their goal body weight a day, spread over 3 to 4 meals. If that feels like a lot, start by getting 25 to 30 grams at each meal. Read how much protein women need for easy ideas.

2. Lift weights 2 to 4 times a week

Strength training is the signal that tells your body to hold on to muscle. You don't need to live in the gym. Two to four full-body sessions a week with squats, hinges, presses and rows, getting a little stronger over time, makes a huge difference.

3. Don't let your calories drop too low

It can be easy to eat very little without noticing. Very low intake speeds up muscle loss and can leave you exhausted. Tracking for a few weeks, even loosely, helps you see if you're eating enough to fuel your body.

4. Eat foods that are easy on your stomach

Nausea and fullness are common side effects. Smaller meals, lean proteins (chicken, fish, eggs, Greek yogurt, cottage cheese), cooked veggies and plenty of water often feel best. Greasy and very large meals tend to feel worst.

5. Keep moving every day

Daily walking supports your energy, digestion and blood sugar, and it's gentle enough to do even on low-energy days. Here's how many steps to aim for.

Think about life after the medication

Many people eventually lower their dose or stop. The habits you build while you're on a GLP-1, eating enough protein, lifting and moving daily, are what help you keep your results afterward. Use this time to build the routine, not just watch the scale.

Frequently asked questions

Do I need to strength train on Ozempic?

If you want to keep your muscle, yes. Strength training plus enough protein are the two biggest things you can do to protect lean mass while losing weight.

How much protein should I eat on a GLP-1?

A good starting point for most women is 0.7 to 1 gram per pound of goal body weight a day. Your doctor or a dietitian can help you set the right number for you.

Why am I so tired on a GLP-1?

Often it's from eating too little overall, too little protein, or not drinking enough water. Talk to your doctor if it keeps up.

The bottom line

A GLP-1 can help with appetite, but it can't build muscle or habits for you. That part is still up to you, and I'd love to help.
